When Dr. Stefan Mann founded Synergy Medical Group, his sole focus was exploring ways to improve overall care and communication with patients and their families, no matter what place in their healthcare journey they are. Dr. Mann and his Synergy team take pride in meeting you where you are in this journey and ensuring you not only understand the changes and plan of care, but that you play an active role in determining your care plans and goals. While we round on patients at various healthcare facilities, we also can meet your primary care needs in either our clinic or your home.

Our team offers a wide range of services including routine check-ups, health screenings, disease management, annual physicals, and urgent care needs.

Home Visits

When you’re unable to leave your home, we can bring the care to you. Our providers can do routine primary care visits right from the comfort of your own home. We understand that sometimes recovery and transportation are major barriers to patients obtaining the healthcare they need, and we seek to remove these barriers by coming to you.
