Synergy Medical Group is expanding across Florida and seeking additional physicians for our rapidly growing post-acute division. This role is ideal for hospitalists, internal medicine, and family medicine physicians who want to step into a leadership-track practice outside the hospital — without sacrificing income or clinical complexity.

Position Summary

We’re seeking a physician who isn’t satisfied with the status quo — someone who sees opportunity where others see routine. In this role, you’ll combine clinical expertise with entrepreneurial vision to expand post-acute partnerships and elevate care delivery across our partner communities.

You’ll have the autonomy to build meaningful relationships with facility leadership, grow patient census through trust and collaboration, and directly influence how care is delivered within the post-acute setting.

Ideal Candidate

  • Entrepreneurial physician motivated to build and scale within post-acute care
  • Relationship-focused leader who excels at building trust with facility decision-makers
  • Proactive, business-minded, and comfortable engaging with facility teams to grow census and strengthen market presence
  • MD or DO with an active Florida license (or eligibility)
  • Background in Hospital Medicine, Internal Medicine, Family Medicine, PM&R, or Geriatrics
  • Demonstrates strong clinical judgment, independence, and accountability
  • A collaborator and innovator with a growth mindset and passion for developing new programs

Required Qualifications (Screening Criteria)

To be considered for this role, candidates must meet ALL of the following:

  • MD/DO only — NP/PA applicants will not be reviewed
  • Active Florida medical license or eligibility to obtain one
  • Experience with medically complex adult patients
  • Clinical background in Hospital Medicine, Internal Medicine, Family Medicine, PM&R, or Geriatrics
  • Strong clinical autonomy and comfort rounding independently
  • Ability to communicate effectively with SNF/ALF/IRF staff
  • Professionalism, reliability, and accountability
  • Interest in leadership, census growth, or program development

Your Role

  • Provide care in SNFs, ALFs, and Inpatient Rehabilitation Facilities
  • Round independently with full clinical and administrative support
  • Manage post-acute and chronic care plans
